Hi- I also have a 19 mo. old who gets a red, dry "rash"-type reaction around his mouth. I think it's from the moisture (spit, mainly) and is intensified after eating, wiping the area, and using a pacifier. We have limited his pacifier use to only when he's in bed sleeping. Also, we use a Heavy Wipe wettened with warm water to wipe his face after meals. They are nice and soft and can be easily machine washed over and over. We also put Vaseline cream on the area two times a day. Be careful not to use anything on this area that may be harmful if swallowed. I wouldn't use Hydrocortisone, for instance. I wonder if it may be an allergic reaction to acidic foods? Does it happen after he eats pasta sauce or ketchup, by chance? Oh, I try to dab the area instead of wiping... Hope this helps a little!!
